How to Use This Kidney Report Reader

What Is eGFR?

eGFR (estimated Glomerular Filtration Rate) is a calculated measure of how well your kidneys are filtering waste products from the blood. It is calculated from serum creatinine together with age and sex. eGFR is reported in mL/min/1.73 mยฒ and is the most widely used indicator of kidney filtration function.

What Is Creatinine?

Creatinine is a waste product produced by muscles and removed by the kidneys. When kidney function is reduced, creatinine builds up in the blood. However, serum creatinine alone is not the best measure of kidney function because it is affected by age, sex, muscle mass, diet and hydration. eGFR, which adjusts for age and sex, is more reliable.

What Is Urine ACR?

Urine ACR (albumin-to-creatinine ratio) measures the amount of albumin (a protein) in the urine relative to creatinine. A small amount of albumin in urine can be an early sign of kidney damage โ€” especially in diabetes and high blood pressure โ€” even when the eGFR is still normal. ACR is usually more useful than a simple urine albumin result because it adjusts for how concentrated the urine sample is.

eGFR and Albuminuria Together

KDIGO 2024 classifies CKD by Cause, GFR category and Albuminuria category. Using eGFR and ACR together gives a much better picture of kidney health than either test alone. A high ACR can indicate kidney damage even when eGFR is above 60. Similarly, a reduced eGFR with a high ACR indicates higher risk than reduced eGFR alone.

Chronic kidney disease (CKD) is defined as abnormalities of kidney structure or function, present for more than 3 months. A single abnormal result alone should not be used to diagnose CKD without confirmation on repeat testing.

When to See a Doctor

Seek medical review if your eGFR is below 60, if your urine ACR is in the A2 or A3 range, if you have diabetes or high blood pressure and any abnormal kidney result, or if you have unexplained swelling, reduced urine output, or other symptoms suggesting kidney problems.

eGFR is calculated from serum creatinine and may be less accurate in certain situations, including very high or very low muscle mass, high protein diets, acute illness, dehydration, recent heavy exercise, certain medications, and in people at extremes of age. In these situations, your doctor may order additional kidney function tests such as cystatin C-based eGFR or a 24-hour urine creatinine clearance.
