Fatty Liver Risk Calculator

Uses the Fatty Liver Index — FLI. Estimates fatty liver likelihood using BMI, waist circumference, triglycerides and GGT. Doctor-reviewed. Free tool.

🔬 What does this tool do?
This tool estimates the likelihood of fatty liver using the validated Fatty Liver Index — FLI.

Fatty liver means excess fat has built up inside the liver. It is commonly linked with obesity, diabetes, high triglycerides and metabolic syndrome.

This calculator does not diagnose fatty liver. It helps identify whether your result is low, intermediate or high likelihood — so you know whether further assessment may be needed.

You will need four things from your reports:
  • Height and weight — to calculate BMI
  • Waist circumference in centimetres
  • Triglycerides from your lipid profile or blood test
  • GGT from your liver profile or LFT report

Understanding the Fatty Liver Index (FLI)

The Fatty Liver Index is a validated, simple score that estimates the likelihood of fatty liver using four routine measurements. Here is what the score means and how it is used.

What is fatty liver?

Fatty liver, also called hepatic steatosis or NAFLD (non-alcoholic fatty liver disease), occurs when too much fat builds up inside the liver. It is closely linked with obesity, type 2 diabetes, insulin resistance, high triglycerides and metabolic syndrome. In early stages, fatty liver may cause no symptoms. Over time it can progress to liver inflammation, fibrosis (scarring) and cirrhosis.

What is the Fatty Liver Index (FLI)?

The Fatty Liver Index is a validated mathematical score using BMI, waist circumference, triglycerides and GGT. It produces a score from 0 to 100. A score below 30 helps rule out fatty liver, while 60 or above suggests a higher likelihood. The FLI is widely used as a non-invasive screening tool because it requires only routine measurements — no ultrasound or biopsy is needed.

FLI score interpretation

FLI ScoreCategoryWhat it suggests
Less than 30Low likelihoodFatty liver is less likely based on these values. Maintain a healthy lifestyle and monitor risk factors.
30 to 59Intermediate likelihoodResult is not clearly low or high. Fatty liver may be possible. Medical review may be useful.
60 or aboveHigh likelihoodHigher likelihood of fatty liver. Medical review and further assessment are recommended.

What should I do if my result is high or intermediate?

A high or intermediate FLI does not confirm a diagnosis of fatty liver. It suggests that further evaluation may be helpful. This may include a liver ultrasound, liver profile blood tests, blood glucose and HbA1c tests, and cardiovascular risk assessment. If fatty liver is confirmed, treatment focuses on weight reduction, diet improvement, physical activity, and management of diabetes and high triglycerides.

Why does GGT affect the score?

GGT (gamma-glutamyl transferase) is a liver enzyme that is often elevated in fatty liver disease. It is sensitive to liver fat, alcohol use and bile duct problems. The FLI uses GGT as a marker because it is a reliable indicator of metabolic liver stress. However, GGT alone is not specific — it can also rise due to alcohol, certain medications and other liver conditions.

Frequently Asked Questions

Common questions about fatty liver and the FLI score, answered by Sineth Hospitals.

The Fatty Liver Index is a validated score that estimates the likelihood of fatty liver without needing an ultrasound or liver biopsy. It uses four routine values — BMI, waist circumference, triglycerides and GGT — to calculate a score from 0 to 100. A score below 30 helps rule out fatty liver, while 60 or above suggests a higher likelihood.
No. This calculator estimates the likelihood of fatty liver. It does not provide a confirmed diagnosis. Fatty liver is usually diagnosed by liver ultrasound or imaging. A doctor must interpret any abnormal result and arrange appropriate tests.
Triglycerides are found in a lipid profile or individual triglyceride blood test — written as Triglycerides or TG. GGT is found in a liver profile or LFT report — written as GGT, Gamma GT or Gamma-glutamyl transferase. Enter the number and select the correct unit shown on your report.
Choose the unit shown on your blood test report. Sri Lankan laboratories commonly report triglycerides in mg/dL, but some use mmol/L. Check the unit label next to the triglyceride value in your report. This calculator automatically converts mmol/L to mg/dL for the formula.
A low FLI score makes fatty liver less likely but does not rule it out completely. Some people with lean or normal-weight fatty liver may have a lower FLI. If you have abnormal liver tests, symptoms or significant risk factors, discuss with your doctor even if the FLI score is low.
Yes. GGT levels are often raised by alcohol use, which can increase the FLI score even without metabolic fatty liver. This calculator estimates overall fatty liver likelihood but cannot distinguish between metabolic fatty liver (NAFLD) and alcohol-related liver disease (ALD). If you drink alcohol significantly, medical assessment is recommended.
A high FLI suggests a higher likelihood of fatty liver and warrants medical review. Your doctor may arrange a liver ultrasound, liver profile, lipid panel, blood glucose and HbA1c tests. If fatty liver is confirmed, management focuses on weight reduction, a healthy diet, regular physical activity and treatment of related conditions such as diabetes and high triglycerides.
